13. Impact of Collaborative Provider Engagement for People Living with HIV who are Virally Unsuppressed despite Consistent Refills of Antiretroviral Medications

Amida Care (AC) is a Medicaid Special Needs Health Plan that serves over 9,000 patients in New York City, of which 70% are People With HIV. In June 2022, AC's clinical pharmacy service established a Viral Load Suppression ARV Refill (VLSAR) Initiative that takes an integrated comprehensive approach to address adherence concerns and improvement towards viral suppression. For 2023, 121 members were identified and evaluated for VLSAR, of which 52% achieved a viral load result of <1000 after pharmacist intervention and provider engagement from VLSAR. For 2024, 105 members were identified for VLSAR and 47 (45%) patients achieved a viral load result of <1000 after pharmacist intervention and provider engagement. 32 (30%) patients experienced a change of ARV regimen after intervention. Provider responses were received for 92 patients from the original cohort.
