Polypharmacy is a growing concern in people aging with HIV (PAWH). This intervention integrated clinical pharmacists into a comprehensive primary care clinics for persons living with HIV, with a focus on PAWH. Clinical pharmacists participated in clinic visits and conducted asynchronous chart reviews with the goal of providing patient education, ensuring medication adherence, avoiding drug-drug interactions, and reducing polypharmacy.
