Social isolation and loneliness impact several intersecting physical and mental health conditions. Social connection has been found to mitigate this impact and lead to positive patient outcomes. This poster overviews the formation of an Advancing Positive Social Connection Collaborative to develop and implement a social connection quality improvement project. This project aims to advance social connection in healthcare settings through quality improvement methodology and evidence-based interventions, ultimately to improve health outcomes.
