New York State’s success in reaching ETE goals in large part depends upon the ability of providers to address the social drivers that serve as barriers to HIV care and treatment. The panel will discuss EngageWell IPA’s “Care Your Way” Pilot, an integrated care model consisting of evidence-based interventions that address common barriers to viral load suppression, including food insecurity, medication adherence, a lack of internet and smartphone access, digital illiteracy, and access to healthcare providers. To achieve this level of wraparound support, EngageWell has facilitated public/private partnerships with 14 community-based healthcare providers, an HIV Special Needs Plan, telemedicine platforms, internet equipment providers, a medication adherence app, medically tailored meal providers, a healthy food and nutrition education app, wireless providers, and a closed-loop referral system.
